To be fair I completely forgot Scott had mentioned M*A*S*H* but I don't see why it couldn't be broadcast in Japan but then I've only ever seen a handful of episodes so that's probably why I'm missing what's obvious to you.
I'm not sure about television in the 60's but I know that T.V. writers, producers etc. want their shows to be sold to other countries as that will increase their profits, case in point the recent Captain America movie: He wasn't fighting Nazis in WWII because movies in Germany aren;'t allowed to show Swastikas. The hand thing is just something they can keep in the back of their mind while filming, it doesn't effect who they hire to play the role (Since they still hired Jimmy Doohan), it doesn't effect the plots and it's not very noticeable (or at least I've never noticed it and I know it's been mentioned before on TTF).
Of course it may have nothing to do with BS&P and may simply be the actors were self conscious about their lack of digits and asked if they could be filmed in such a way that didn't call attention to the fact.
